Consent to Provide and Disclose Personal Information(Required)

I agree that Training Centre Canada on behalf of The Smith Investigation Agency Inc., or Smith Security Inc. (training entity) to collect personal information from or about me for the purposes of providing training in accordance with the Training and Testing Regulations made under the Private Security and Investigative Services Act, 2005 (PSISA).

I consent to disclose personal information collected from me or about me, including whether or not I have successfully completed the required training under the Training and Testing Regulations, to the Private Security and Investigative Services Branch of the Ministry of Community Safety and Correctional Services for the purpose of determining whether I am eligible to be licensed as a security guard or private investigator and for the purpose of administering the licensing system authorized under the PSISA.

I also consent to and authorize the Private Security and Investigative Services Branch of the Ministry of Community Safety and Correctional Services to disclose personal information collected about me for the purpose of advising the Ministry's Test Delivery Vendor as to whether or not I have completed the required training under the Training and Testing Regulations.

I hereby release and discharge His Majesty the King in the Right of Ontario, Training Centre Canada, The Smith Investigation Agency Inc., and/or Smith Security Inc., and their respective directors, employees, subcontractors, volunteers, servants, and agents, including their successors and assigns, from any and all actions, claims, and demands for damages, loss, or injury whatsoever arising, except as a result of negligence or willful misconduct, which may thereafter be sustained by myself as a result of the collection, use, and disclosure of the personal information as authorized by this form.

This release of liability shall be binding upon and shall ensure to the benefit of my respective heirs, executors, administrators, and assigns.

To complete your training and receive your TCN (Training Completion Number), you are required to upload a screenshot of your course dashboard clearly showing 100% completion. This step is mandatory and ensures that you have fulfilled all Ministry-required training hours. The screenshot must include your name and the visible 100% completion status. If your dashboard does not show 100% completion, you must finish all required modules before submitting this form.
